Audit Trail

An audit trail links recorded actions to relevant actors, objects, times and outcomes so a reviewer can reconstruct a decision or change. It is a purposeful evidence record, not necessarily every debug message. Completeness, retention, access controls and resistance to alteration determine its usefulness; an entry alone does not prove the underlying claim is true.
